Chapter 1: Introduction to VR in Pain Management

The simple answer to whether virtual reality (VR) can assist individuals in managing pain is a resounding yes. However, the extent of relief that it can deliver might come as a surprise.

During my time working with a VR setup for almost a year, I had the opportunity to engage with a sophisticated system that surpassed typical home VR setups. This robust configuration included an HTC Vive headset, a custom-built PC featuring an i7 7700K processor, 32GB of RAM, an RTX 2070 graphics card, and an SSD for storage. The designated play area was roughly 20 x 20 feet.

My first encounter with VR was unforgettable. I transitioned from painting in a 3D environment to walking a plank high above the city and spending hours in a garage alongside Rick and Morty. I journeyed through Antarctic landscapes, explored the Amazon rainforest, battled zombies in a dystopian sewer, and even soared into outer space, enveloped by the beauty of the cosmos. Those were truly remarkable experiences.

Throughout our time hosting VR sessions for around eighty participants at our company, witnessing newcomers immerse themselves in these virtual realms was just as exhilarating as participating myself. To enhance the experience, we set up a monitor displaying what the players were seeing. Most individuals enjoyed the experience, though some found it challenging. The motion sometimes induced nausea, and for others, the realism of the virtual world proved overwhelming.

One memorable moment involved a participant who, while submerged in a virtual shipwreck, shouted for an exit—not because of the sharks or whales gliding by, but due to the claustrophobic sensation of being deep underwater.

Interestingly, nearly every session concluded with players speculating on alternative applications for VR technology. Many quickly envisioned its potential use in our market research efforts. The concept of mapping grocery store layouts for a shopping experience or showcasing new products, such as vacuum cleaners or vehicles, made perfect sense.

Pioneers in various industries are beginning to explore the potential applications of VR technology. It represents an exciting frontier that will undoubtedly demand the usual cycle of trials, successes, failures, and financial investments before achieving optimal solutions that provide tangible returns.

Chapter 2: VR's Role in Healthcare

In terms of practical application beyond entertainment, VR has an ideal fit in the medical field. Significant progress has been made, backed by substantial data supporting its use in pain management and physical therapy.

The FDA does not typically approve new treatments lightly. In November 2021, it authorized VR as a treatment option for alleviating chronic lower back pain, a condition that nearly everyone will face at some point in their lives.

For Immediate Release: November 16, 2021 — The U.S. Food and Drug Administration has authorized the marketing of EaseVRx, a prescription-only immersive VR system employing cognitive behavioral therapy and other methods to aid pain reduction in patients aged 18 and older with chronic lower back pain.

Chronic lower back pain, characterized by moderate to severe discomfort lasting longer than three months, is one of the most prevalent chronic pain conditions in the United States. This type of pain can restrict mobility, interfere with daily life, and is often associated with anxiety, depression, poor health perceptions, decreased quality of life, and reliance on opioids.

The distraction provided by VR has been shown to help individuals cope with traumatic and painful experiences. At the University of Washington Medical Center, researchers instructed participants to engage in a game called Snow World within a VR setting while receiving mild electric shocks. Results indicated that they experienced significantly less pain, with reports showing pain ratings reduced by as much as 35 percent, compared to just a 5 percent reduction for those who listened to music as a distraction.

Similarly, at Brooke Army Medical Center, physicians treated Lieutenant Sam Brown, who sustained severe burns after his vehicle was hit by an explosive device in Afghanistan in 2008. Despite being on heavy pain medications, Brown reported minimal relief. However, while engaged in Snow World, his pain rating dropped from the maximum of 10 to a 6.

Notably, during his time in Snow World, soldiers reported thinking about their pain only 22 percent of the time, a stark contrast to the usual 76 percent without VR.

Additionally, VR can facilitate patient engagement in hypnosis. By loading pre-recorded hypnotherapy sessions through the VR headset, patients can experience tranquil settings with soothing guidance on relaxation techniques aimed at pain reduction.

Research by psychologist David Patterson on various patients with broken bones and gunshot wounds revealed that VR hypnosis can be more effective than Snow World in minimizing daily pain increases.

The mind remains an enigmatic force capable of healing various ailments. By investigating emerging technologies like VR alongside established practices such as hypnosis and biofeedback, the medical field can potentially innovate healthcare and deepen our understanding of the body-mind connection.
